It’s usually best to get a lawyer to help you fill out your application. To speak with a lawyer for free advice or to get legal help with you situation, contact Justice for Children and Youth at 1-866-999-5329 or at www.jfcy.org.

A Rights Adviser can also explain the application form to you and help you fill it out, but it’s best to get a lawyer’s help. If you need help finding a Rights Adviser, you can contact the Psychiatric Patient Advocate Office at 1-800-578-2343.

Application forms are available on the CCB website. It doesn’t cost any money to submit an application.

CCB questions about being an informal patient

If you’re 12 to 15 years old and you’re asking the CCB to review whether you must stay in the facility as an informal patient, you should think about how you’ll answer these questions that the CCB may ask you:

• Can the psychiatric facility give you the treatment that you need?
• Can you get the treatment you need without being in the facility?
• Are there any other options to the psychiatric facility?
• Where would you prefer to live?

CCB questions about your capacity to make decisions

If you’re asking the CCB to decide whether you should be allowed to make your own health-care decisions, you should think about how you’ll answer these questions that the CCB may ask you:

• Do you understand the mental health diagnoses that you have?
• Do you understand the type of treatment the doctor wants you to get?
• Do you understand what will happen if you don’t get treatment?
• What are the reasons you want or don’t want to get the treatment?
