2. Give your forms to Service Canada

After you fill out, print, and sign the form, you must take it to a Service Canada Office or mail it to a regional Service Canada Office. The mailing address for Ontario is:

If you mail the application, it might take several days to arrive at Service Canada’s office. You will need to leave extra time to make sure your application is not late.
