Sample survey / website feedback questions

Below is a sample of the survey functionality and sample questions that could be added to a website feedback form. Results will be tabulated in the backend of the website and can also be exported to Excel.

There are lots of options for the types of questions and fields that can be added to a form and additional options such as adding columns, multiple pages and conditional logic (displaying additional or different questions based on how someone answered a question).

Sample ratings

The following fields are different types of "ratings fields" which include options to set a scale from 2-10 and select a shape (star, heart, smiley face or thumbs up) then set a size and colour. We could further customize it if needed (such as showing circles with a # value) but currently are just showing the defaults/out of box options.
