2. Apply for post-adoption birth information

If you are at least 18 years old and you were born in Ontario, you can apply for Post-Adoption Birth Information.

To apply for Post-Adoption Birth Information you must fill out a form called an Application for Post-Adoption Birth Information. It is also called form 11315E.

You have to give as much information as you can about yourself both before and after your adoption.

You may be able to get:

• your original birth registration, which may have your birth parents’ names
• your first and last name at birth
• the place where you were born

If you were adopted before September 1, 2008 your birth parents may have applied for a disclosure veto. This means that any information that would reveal their identities will not be released to you. It might even say why they don’t want their identities to be released.

Your birth parents may have also filed a no contact notice. This means that you have to sign an agreement promising not to contact your birth parents before you can get identifying information about them. If you sign the agreement and then try to contact your birth parents, you can be fined up to $50,000.
